Help, CC Debt & Totally Disabled in NY

I am at wits end. I will be receiving SSD for a chronic debiliating condition, and that will be all my monthly income will be. I have substancial CC debt with 4 cards. I am up to date, but the my savings is gone due to medical insance and trying to pay off this debt, can no longer afford to pay. I also have been borrowing from one card to pay the others. I own a condo, up to date with Mortage always. I do not feel there is any way I could possible pay my outstanding CC debts as of today, and afraid I will loose my house. What can I do? I can't even afford a bankrupsy lawyer and not sure I want to go that route. I read on another post about a letter to send creditors to cease comminications, where do you get this letter?
