It looks like your account has been sold twice, possibly three times. By the time an account gets sold, resold, and close to SOL, they usually don't bother to keep the paperwork from the OC because they are counting on scaring you into paying or filing a lawsuit and you not showing up in court so they get an easy default judgement. I wouldn't loose too much sleep over this if it were me. Do what Whychat said and check your SOL on the debt by the standards his site has listed. I wouldn't hide from them if they do file suit and I would show up so they can't get an easy default judgement. Once they go to court and you show up they have to prove their case and without any documentation, that's impossible.
