BTW, a guarantor is a person who says "if this other person doesn't pay it, I will." So if he did sign it he is legally liable, if he didn't he is not. He really should find out if he is trying to get out of paying this. If he feels his moral obligation, it doesn't matter. If the hospital is fair with him and doesn't start adding on interest and fees which would make paying off the debt impossible, then paying it is the decent thing to do.

He should check his credit reports to see what is being reported.
