How do they Validate Credit Card and Utility company debts?

How can a Collection agency validate a credit card debt or utility debt? There are no signed documents with utilities at least. Some credit cards can be obtained by applying via telephone. Utilites don't make you sign a document do they? I guess I am wondering how does a collection company validat a debt to Pacific Bell for example.
