Razor, thank you for your suggestion, I did contact the original insurance co, but they no longer have the records because the company went out of business and the records were destroyed. They do however know that the company terminated their service as of Aug 1 1996. So I should have been covered. When I talked to the CA, I was told that since the unpaid bill is so old, the insurance would not pay for it.
