Given the large amount of $$$ involved, you might want to consider hiring an atty that specializes in credit/collection cases.
However, since it has gone to collection, you can make the FDCPA work for you. Validate! You've asked where those addt'l charges came from--but you need to do it in a more formal, written and stern manner--making the CA well aware that you are aware of your protected rights under the FDCPA--then the HAVE to explain EVERY charge to you.
I'd spend a lot of time studying these boards and learning everything you can about the FDCPA--also thoroughly check your state laws (esp. in re medical collection) AND the statute of limitations on med collections. If you've got the money to settle the original amount, I think you'd be in good shape, however, I'd make sure that the CA was playing by the rules by validating first. When they know you are serious, they may be more willing to settle. Don't accept settlement or pay anything without: EVERYHING in writing--agreement that acct is paid in full, that reaminder of acct balance will not be sold/collected at any time and DELETION from your CR's.
