I believe a phone bill would be an open-ended account as there are no set terms as to amounts, payments, etc.

So, given that and if you last paid the thing in 1996, the SOL has expired. You don't have to pay this. Send the collection agency WhyChat's expired SOL letter, they can't sue you for it any more and they can't force you to pay anything. If you're going to file bankruptcy make sure you include this debt.

If you're really going to file BK, don't waste any more money on the credit cards or the counseling service - its money down the drain. Keep it to pay a bankruptcy attorney.
