Jasper, look again at the section you posted. Gramatically speaking, I agree that the statute is somewhat poorly written in this way, but what it is saying is that if the debtor *requests* the name and address of the original creditor, the CA has to provide it. It also says that if the debtor disputes the debt, the CA has to provide verification of the debt. There is case law a-plenty as to what does and doesn't constitute proper verification. The "name and address of the original creditor" part and the "verification" part are two separate aspects of the statute.
