Someone else may be using your ssn, or your report may have been merged with someone else, or the cable company miskeyed the inquiry.

To get your correct address information listed, and dispute the incorrect address, go to the research request form and fill out the information as it asks below

“If your identifying information differs from the information listed on this form, please fill in the correct information in the space provided below each item. Please provide a photocopy of your driver’s license, social security card, or a recent utility bill that reflects the correct information”

You can dispute the DTS-Comcast inquiry as no permissible purpose. (Make sure Comcast doesn’t have some account opened with your credit, if the person skips, they’ll tag your credit with a collection)

You can dispute the bank inquiry also, but they may come back stating you had a relationship with them, then you’d have to dispute directly with your bank over why they pulled a credit report on you.

Check your Experian and Trans Union reports also. When one has bad information, they all usually do.

You may want to put them all on fraud alert status to prevent any new accounts being opened on your credit.
