Default Judgement

I owned a Condo in 1996 which was foreclosed on, this left unpaid condo fees for which a judgement was entered. I filed for bankruptcy and had it discharged in 1997 and included the judgement in the bankruptcy. I have since bought another home which I am trying to refinance. The refinancing lender is telling me that when they did a title search on my present home the judgement has been attached to my present title. I have received documentation for the condo association stating that it had been satisfied thru the discharge of the bankruptcy. But the judgement is still showing up on title of my present home. Is it possible for a judgement on previous property, especially one that has been included and discharged in a bankruptcy, to follow and be attached to property that had nothing to with the judgement in the first place?
