"Combined" is sort of a loose term. When one agency represents a hospital, any account that comes to that agency after receipt of the original may be added together to simplify bookkeeping, and to make sure agencies aren't calling the same person for 8 different date of service bills in a short period of time. You can always obtain itemized bills for certain patients or dates of service. You just have to request these in writing since the HIPPA laws have gone into effect for all companies, clinics, offices, and agencies that deal with anything medical.
