Ok, I have two teen-aged step-children as well, so I can tell you how this works. :-)

If the hospital made a claim on your b/f's insurance and he is the custodial parent, they will go after him. Whether the ex-wife was liable under a support order is immaterial as far as the hospital is concerned. If he can't get his ex-wife to pay her portion as stipulated by the support agreement, *he* will have to go to the court to get it enforced.

The support agreement is between your b/f, his ex-wife and the court, not between the hospital and his ex-wife. And for the sake of harmony, it's usually best to leave such matters strictly between the parties involved. Just so you know, most family courts see involvement of significant others, even current spouses, as "interference" that they view with a jaundiced eye. It doesn't even matter if you're the primary breadwinner of the household.

My advise to you, which of course you are free to decline, would be to advise your b/f of his options, then leave it be. It will save you a lot of heartache and frustration.

Good luck!
