Yes, I did a receive a summons on 3 different occassions, each with the same date on them. As soon as I received them, I either called or emailed Consumer Legal Centers, who in turn had me immediatly fax them a copy for them to handle it. And as you stated, it was somewhat "official" looking with "Cohen & Slamowitz" typed in the letterhead and under my inside address the words " blank City Court, County of blank, State of New York". But it never stated any date or time when I was supposed to show up at court. Just that I did not have to appear in person. It was then "signed" by David Cohen, esq.
