I'm also in Cook County - I found their website is not really detailed enough re the rules. If you can get a copy of the Sullivan's Law Directory all of the rules are set out in it - including Supreme Court, Circuit Court, etc. Or call the clerk's office - they are usually pretty helpful in telling you specifics of dates, forms, etc. If you go to room 601 of the Daley Center to the Motions desk you will find out everything you need to know - just tell them you are filing pro se and they are very helpful.
