Answer letter forms

I need to submit an answer to a civil summons. Does the court provide the forms to answer. If they do, I didn't get one. How am I suppose to make one? I saw the one from whychat- but it is broken up. Are they trying to get me to not answer, because as a normal person, I find this a very hard process to do with only 28 days left. Help!
