CA did hard inquiry

I did the HIPAA letter with my HC Provider. They agreed to take care of the CA and get them off my back. The account has been paid in full to the OC. I recently disputed the account with Equifax and they came back as verified by the CA. Also, I notice that the CA did a Hard Inquiry this month for my credit report without my authorization. What is my recourse?
