BK ?s - need advice

I am thinking of filing BK, but I am unsure how you decide if to file Chap 7 or Chapt 13 or does the attorney tell you which to file. If I am understanding it right with Chapt. 7, we are completely relieved of our debts but with Chapt 13 we pay back in 3-5 years. Is that right? We live in SC own a house and an old car and truck and am curious what would happen. Just wanted to get a feel for it before I made an appt to talk with an attorney. Does it take care of medical bills, too? I know you have to go before a judge but what reasoning would they have not to allow you to file Chapt 7 and be discharged from all of your unsecured debts?
